Clinical history:
A 43-year-old female patient with chest pain for two months. Pain with a maximum intensity of 8-9/10 on minimal exertion, worsening on inspiration and involving both shoulders. A cough with clear phlegm and hoarse voice in the last two days. An additional finding was that of a swollen right index finger.
